Immunization Records in Vermont

The Vermont Immunization Registry (IMR) is a secure, centralized health information system containing Vermonters' immunization records. Medical providers and school nurses use the IMR to find patient histories, print IMR reports and report coverage.

Update

The Immunization Registry Team is expecting updates to Automatic HL7 messaging (VXU) in Spring 2026 to assist VDH and other constituents with required reporting and vaccine ordering practices. This will include accepting updates, edits, and deletions from a practice’s EHR, and Vaccine National Drug Codes (NDC).

There will be additional communications to follow regarding this change in the coming weeks and months. Print and save our One-Pager for more information.

Tips for Logging In and Password Requirements

As of September 2024, any new passwords must contain:

  • a minimum of 14 characters
  • numbers
  • a combination of uppercase and lowercase letters
  • special characters (& * % @ ...)
  • and cannot include the user's first or last name

Information for Health Care Providers

Healthcare providers who administer immunizations or need access to immunization information for the purpose of delivering health care may have access to the IMR. Medical providers may apply for an IMR account by completing the Confidentiality Agreement below. Contact IMR user support at 888-688-4667 or [email protected].

Important: only Trivalent Vaccines have been approved for the 2025-26 Influenza Season. Please make note of this when reporting flu vaccine given and refer to our Influenza Code Guidance if you have additional questions.

Practice Level Reports

Practice level reports are available to Registry users from primary care practices that patients are assigned to. These reports are not available to School or Licensed Childcare users, or facilities that give vaccine but do not have assigned patients, such as pharmacies and specialty clinics. To get started using practice-level reports, first view our Introduction to Reports user video.

1.2 Practice View: Vaccine CoverageAn assessment of vaccine coverage for expected vaccines for the selected age range, including number and percentage of patients up to date out of the total
1.3 Vaccines Due by Practice (user guide)A list of patients considered not up to date for selected vaccine(s), by patient name and date of birth
1.4 Not up to Date ReportIncludes all patients who are not up to date for the chosen vaccine group. The easiest way to find patients negatively impacting vaccine coverage
1.5 Generate Labels for Recall NoticesAllows a user to print out mailing labels for patients who are not up to date for either one or all vaccine groups
1.6 Invalid DosesProvides a list of all instances of invalid doses, where the vaccine given does not match the accepted guidance, for all patients within the selected age range
1.7 Objections and Contraindications for PracticeProvides a list of documented objections and contraindications to vaccine for all patients within the selected age range. Also included: history of Varicella disease
1.8 Immunizations Given by PracticeA list of every vaccine reported to the registry within the selected date range, sorted by age brackets

2.2 Immunization Quality Improvement Report (user guide)A comprehensive representation of Immunization rates for a practice. It assesses vaccine coverage, patients not up to date, and invalid doses for a set of standard age ranges within the practice

Information for Schools and Child Care Providers

School Users

School nurses and administrators at schools without a school nurse on staff may have access to the IMR for the purpose of completing the immunization coverage reporting that is required each year. School nurses and administrators may apply for an IMR account by completing the Confidentiality Agreement below.
