VFC & VFA Vaccine Accountability & Order Forms
The Centers for Disease Control and Prevention (CDC) requires all states to use the vaccine tracking system, VTrckS, to place, track and manage vaccine orders on-line. All Vermont VFC/VFA providers must enroll in the new system and order state-supplied vaccine online; therefore, VFC & VFA vaccine accountability and order forms are no longer provided by the Vermont Immunization Program.
