CHAMPPS: Coordinated Healthy Activity, Motivation & Prevention Programs

Healthy Communities"

Communities throughout Vermont are building and implementing comprehensive health and wellness projects that promote healthy behavior - including better nutrition, increased physical activity, and disease prevention.

To fund these projects, the Department of Health distributes the state's Coordinated Healthy Activity, Motivation & Prevention Programs (CHAMPPS) competitive, multi-year grants to Vermont-based community organizations.

Apply for a CHAMPPS FY2011 Grant

For FY2011, eligible organizations or coalitions are those who are completing year one of the two-year FY2010 CHAMPPS funding.

Resources

The Fit and Healthy Vermonters Community Assessment Toolkit provides the resources necessary to develop programs and services that best address the nutrition and physical activity needs and wants of your community.

Centers for Disease Control and Prevention's Community Health Assessment and Group Evaluation (CHANGE) Tool assists community teams (such as coalitions) develop community action plans by walking team members through the assessment process and providing summary data about current policy, systems, and environmental change strategies.
