The Provider’s Toolkit is a project of the Vermont Department of Health, and was developed in cooperation with community partners including the Vermont chapters of the American Academy of Pediatrics and Academy of Family Physicians.

The Toolkit offers to providers of pediatric care an organized compilation of current best practice recommendations and resources related to preventive health supervision and health screening for children and adolescents. It incorporates and supports the Vermont Health Screening Recommendations for Children and Adolescents (EPSDT periodicity schedule).
