Healthy Vermonters Plans & Reports

Healthy Vermonters 2020 cover - State Health Assessment Plan

With the publication of Healthy Vermonters 2020, we began our third decade of engaging policymakers, government, health and human services professionals, and the public in setting, measuring and working to achieve long term public health outcomes.

Monitoring health data is an essential function of public health, and this is the state health assessment plan that documents the health status of Vermonters. This will guide the work of public health through 2020. This report presents more than 100 public health indicators and outcomes carefully chosen by state government, health and human services professionals, and the public from the many hundreds set out by the Healthy People 2020 initiative to improve the health of the nation.

For the most up to date Healthy Vermonters data, maps and trends, use the Public Health Data Explorer and Healthy Vermonters 2020 Performance Scorecards. Both have topic-specific reports with statewide population data. The scorecards also have performance measures about how the State of Vermont and partner programs are performing, while the data explorer is the primary tool for accessing county, local health office district and hospital service area population data with maps, trends and community profiles.
