Immunization Records in Vermont

The Vermont Immunization Registry (IMR) is a secure, centralized health information system containing Vermonters' immunization records. Medical providers and school nurses use the IMR to find patient histories, print IMR reports and report coverage.

Incorrect Patient Vaccine History Showing

There are currently several issues in our vaccine forecaster logic that may incorrectly evaluate a patient’s vaccine history. 

  • What: Patients' evaluations of up-to-date status maybe listed incorrectly or different than expected. Affected vaccine groups include: PCV20, RSV, COVID-19, DTP, MMR, Influenza, and Varicella. This could also affect practice coverage rates.
  • Why: This is due to a delayed release of an update to the vaccine forecaster logic that has not yet been implemented nationally. 
  • When will this be fixed? We expect to see progress made within the next month or two.
  • Workaround: To evaluate a patient's vaccine schedule, refer to the current CDC Vaccine Schedules, which include drop-down information for expected vaccine groups and unique situations.

Vaccine Record Requests

Individuals or parents of minor children may request a copy of their vaccine records. Refer to the Instructions on How to Request Vaccine Records or see the step-by-step instructions below.

Note: The Vermont Department of Health does not currently provide QR codes or new COVID vaccine cards. Some pharmacies or other organizations offer replacement cards. Please check with the organization where you received your COVID vaccine.

Requesting Vaccine Records

Step One: Verification

Prep your verification materials. 

If you are an adult 18 years or older, you need to send us a picture of a photo ID (driver’s license or a passport), your date of birth, and your current name, as well as any other names you have held in the past. If you don't have a driver's license or passport, call us at 888-688-4667 and choose option 1.

If you are requesting records for a child under the age of 18, you will need your ID and your child’s name and date of birth. If your child is 18 or over, they must complete this process themselves.

Step Two: Submit Your Request

By Email: Emailing your materials and request to [email protected] is the quickest way to send this information. Please have "Records Request" in the email subject line.

Step Three: Getting Your Records

Once we have verified the identity of the patient, we will send the record in an encrypted email, or by mail.

Note: Many patients over the age of 30 may be missing vaccine records pre-1995 in the system. The IMR has been live only since 2004, and some older records have not been entered.

If we do not have your records, contact your primary care provider.

How-to Navigate Encrypted Emails:

  • You will not be able to access encrypted emails on a smartphone. 
  • When using a PC, click the "read message" text in the encrypted email. This will open a new tab in your browser, prompting you to log in to your email. Then, you will receive a second email with a passcode, which you'll use to access your file. If you are still unable to access your file, we can send you a copy in the mail if you provide us with your mailing address.

School Users

School nurses and administrators at schools without a school nurse on staff may have access to the IMR for the purpose of completing the immunization coverage reporting that is required each year. School nurses and administrators may apply for an IMR account by completing the Confidentiality Agreement below.

Data Requests and Data Briefs

Individual information in the Registry is confidential, but we do publish summary data including county level vaccine coverage rates. Health care providers may request information about their own medical practice.

The IMR team publishes regular reports including state and county level information on a variety of topics. These are developed in concert with the Immunization Program and community partners in an effort to provide Vermonters with current, useful information about immunization in our state.
